If you have any questions or concerns about the care your loved one received
from EPUT, please do get in touch with the team who were responsible for
their care in the first instance. Alternatively, you can contact our Patient

If you feel you are not coping with grief and require specialist bereavement support, you can also talk to your GP.
All deaths in England and Wales are independently reviewed either by a local
Medical Examiner (a senior doctor independent of the healthcare professionals
who provided care) or by HM Coroner where they have a duty to investigate
the cause of death. The Medical Examiner reviews all relevant medical
records, discusses the death with the team who were responsible for the care
and the next of kin before issuing the Medical Certificate of Cause of Death
(MCCD). The MCCD is needed to register the death. The local Medical
Examiner’s Office will contact the next of kin and guide them through this
process, including any necessary referral to HM Coroner.
