Summary
Older Peoples Community Mental Health Teams (CMHT’s) support older people requiring specialist mental health services.
CMHT’s are comprised of community psychiatric nurses, occupational therapists, psychologists, approved social workers, support workers, psychiatrists and staff grade doctors.
The teams work with patients in their own homes which can include; warden controlled complexes, private and local authority residential homes, private and NHS nursing homes, and the Trust’s community resource centres.

Service information
CMHT’s perform a variety of working including:
- Assessment
- Care planning, co-ordination and monitoring
- Rehabilitation
- Occupational therapy
- Domiciliary support
All initial assessments are carried out by a qualified worker and if ongoing care is required a care co-ordinator is assigned.
Please contact the ward before your visit, so they know you are coming, allowing them to communicate this to staff and ensure you are admitted without delay.

Referral information
Referrals are made by GPs, Community Mental Health Teams or the Accident and Emergency Department.
Referrals are also accepted for people under the age of 65 who have early onset dementia.
