Health Outreach Service

Summary

Health Outreach supports marginalised and vulnerable adults into mainstream health services across Suffolk. This includes:

• People experiencing homelessness
• Refugees and people Seeking Asylum
• Gypsy, Traveller and Roma populations
• Vulnerable migrant workers
• Offenders & Ex-Offenders

Inclusive of all black and minority ethnic groups.

We are a multi-disciplinary team made up of general and mental health nurses, social workers, outreach practitioners, support staff and a qualified psychological wellbeing therapist for Refugees.

Service information

We have a flexible approach to reaching out to our clients. We meet them in their own surroundings, which could be their homes, hostels, on the street, or in community partnership settings.

Our aim is to engage clients with mainstream health services, advocate for clients within these services to ensure their needs are met and they understand how the health system works. We provide practical interim health interventions, such as wound care, vaccinations, foot care, assessments and support to access mental health services.

We do not have direct access to accommodation.

Partnership working

We are fortunate to have strong local partnerships across the county within a multitude of statutory, voluntary and charitable sectors that all have supportive roles in the work we do and services we provide to all our vulnerable clients.

Our service is a valuable resource for the Communities of Suffolk and is a unique provision. Every client is treated as an individual through collaborative care planning, holistic assessments, support and care.
